Inspectorate Office and courtesy copies are sent to related personels.] By investigation, we found that [Jamie Sanchez] unauthorizedly used toxic and unverified hazardous experimental materials for self-experiments without appropriate authorization, risk assessment, nor safety protection procedures. This behavior is a major violation the safety regulations of the Research and Development Department, a major violation of personel safety, and has potential adversely affect on the management of experimental materials and the team's research regulations. The U.R.C. Inspectorate Office hereby issues a public reprimand to [Jamie Sanchez], ordering said person to immediately cease all relevant activities, submit a written reflection on the event, and undergo laboratory safety training.
